This information includes your name, address, and what your residence status is and amount of time at that residence. Next, you fill out your employment and financial information. It is here that you are able to specify your occupation, your total income, whether or not you intend on using the card at an ATM, whether or not you have a savings or checking account, and how many times you have taken cash advances from your existing cards in the past six months.

The next step is to fill out your contact information. This includes your telephone number and email address. You must also specify that you are a U.S. Citizen. You have to be a citizen and at least 18 years of age to qualify for the card.
